School District of Poynette - New - Athletic Forms and Fees

Welcome Student Athletes!

Your first step to enjoying an organized school sport is to complete the registration process. The Google form, Sign Up for Middle School Sports is the first form you must complete for registration. After completing the google registration form, refer to the links below to complete the other required documentation to participate in athletics.

Student/Athletes will need to turn in the following items:

Middle School students need to complete a Physical or Alternate Year card, sign the Athletic/Activity Code, and complete an Emergency Medical Form. Forms are listed below. All Middle School Sports have a $25.00 fee.

Middle School Athletic Forms

WIAA Physical Card

Your physical card must be completed and returned prior to any participation.
The purpose of this card is to let the school know that a student has been determined healthy enough to participate. This determination needs to be made by a Doctor. So students participating in their first sport or for the first time for the school needs to turn in a green card. This needs to happen every other year. On the off year, an alternate card is all that is required which is signed by the parents. The form is found below.

2. Athletic/Activity Code

This form must be returned by the first game of the year. This will be handed out and explained at the parents’ meeting early in the season. This is board policy and students need to follow it year round and fall under the guidelines of the policy even if they do not sign it.
